Dental Marketing Study in Spain 2025

Real data from 120,000+ leads generated for 40+ dental clinics across Spain. CPL by treatment, channel performance, conversion rates, and sector benchmarks.

Methodology

This study analyzes aggregated and anonymized data from campaigns managed by Dentalead between January 2024 and December 2025. It covers more than 40 dental clinics across 15 Spanish provinces, ranging from independent practices to multi-location groups. The same discipline we require of a clinical record: document everything, measure everything.

  • 120,847 leads analyzed in total
  • €5M+ in managed advertising spend
  • Channels: Meta Ads (Facebook and Instagram), Google Ads (Search, Display, Performance Max)
  • Treatments: Implants, orthodontics, cosmetic dentistry, whitening, general first visit

All cost data reflects direct advertising spend only, excluding management fees and creative costs.

1. Cost per lead (CPL) by treatment

CPL varies significantly by treatment. Higher-ticket treatments typically carry a higher CPL, but their profitability is superior due to higher patient value.

Average CPL on Meta Ads by treatment (€)
Implants
€18
Orthodontics
€12
Cosmetic dentistry
€14
Whitening
€8
First visit
€6
Average CPL on Google Ads by treatment (€)
Implants
€35
Orthodontics
€25
Cosmetic dentistry
€28
Whitening
€18
First visit
€15

The difference between Meta Ads and Google Ads is explained by intent: Google captures existing demand (the patient is already searching), while Meta creates new demand (the patient was not actively searching). Both channels are complementary.

2. Conversion rates by funnel stage

Not all leads become patients. Understanding each funnel stage identifies where opportunities are being lost and allows process optimization — the same principle as a phased treatment plan.

Average conversion by stage
70%
Lead → Contacted
52%
Contacted → Appointment
40%
Appointment → Treatment

This means that from every 100 leads, approximately 14–15 become patients who start treatment. The key is contact speed: clinics that contact leads within 5 minutes have an 85% contact rate, versus 55% for those taking over 1 hour.

3. Ad spend distribution by channel

Optimal channel allocation varies by treatment and clinic objective.

Meta Ads: the volume engine

Meta Ads (Facebook and Instagram) generates the highest lead volume at the lowest cost. Particularly effective for aesthetic treatments (invisible orthodontics, whitening, veneers) where the visual component of the ad drives demand.

Google Ads: purchase intent

Google Search captures high-intent patients. Although CPL is higher, the patient conversion rate is 35% above Meta. It's the priority channel for implants and high-ticket treatments.

4. Benchmarks by clinic size

Metrics vary significantly by clinic size and digital maturity.

Average monthly ad spend by segment
Small clinic
1-2 chairs
€1,500
Medium clinic
3-5 chairs
€3,000
Large clinic
6+ chairs
€6,000
Dental group
Multi-location
€12,000

Small clinics generate 30–60 monthly leads, medium clinics 60–120, and large clinics exceed 150 leads/month. Average ROAS ranges from 8x to 25x depending on treatment mix.

AI in lead management

Clinics that integrate AI-powered response (chatbots, automated WhatsApp) improve their contact rate by 40%. Response speed remains the single most determinant factor in conversion.

Short video as dominant format

Reels/Stories format ads generate CPL 25% lower than static image ads. Real patient video testimonials are the highest-converting content type.

Google Performance Max

PMax campaigns are maturing and starting to deliver competitive results versus Search for clinics with established conversion history. We don't yet recommend PMax as a primary channel, but it works well as a complement.

The informed patient

73% of patients research online before contacting a clinic. Having educational content (blog, videos, guides) shortens the decision cycle and builds trust. Clinics with an active blog convert 18% more.

Conclusion

Data-driven dental marketing is the primary competitive advantage for independent clinics in Spain. Clinics that measure, optimize, and invest consistently achieve results that position them ahead of chains in acquiring high-value patients.

The key is not to invest more — it's to invest better: in the right channels, with the right messages, and with a follow-up system that doesn't let opportunities escape. Predictability, not luck.
